Rules and Regulations:

1. This competition is open to all uths aged between 12 to 21 years old.
2. All participant(s) have to send a soft copy of their individual or group photos to uthnityatbol@hotmail.com
3. You can use or prepare any props or costumes if needed for your item.
4. Each application would cost a registration fee of $1 (fees collected goes to UthNiTy fund).
5. All participant(s) have to submit this form by 2nd September 2007 (Sunday). Any forms submitted later than that would be rejected.
